Introduction
This policy outlines the commitments and responsibilities of Katie Lourenco, LCSW in providing services to clients. We strive to offer a high standard of care and ensure the well-being of our clients.
Services Offered
- Online Individual Therapy Sessions
Scheduling Appointments
- Appointments can be scheduled through the online booking system or by contacting the Katie Lourenco, LCSW directly.
- Clients are encouraged to book their sessions in advance to ensure preferred times.
Cancellation and Rescheduling Policy
- Clients must provide at least 12 hours’ notice for cancellations or rescheduling.
- Cancellations made less than 12 hours in advance may incur a late cancellation fee.
Payment and Insurance
- Payment is due at the time of service unless otherwise arranged.
- I accept various forms of payment, including all credit cards (Visa, Mastercard, American Express, etc.) and some insurance plans, where applicable.
- Clients are responsible for understanding their insurance coverage and benefits.
Confidentiality Practices
- All client information is kept confidential in accordance with HIPAA regulations.
- Therapist will not disclose any information without the client’s consent, except in situations where there is a risk of harm to self or others.
Client Responsibilities
- Clients are expected to actively participate and engage in the therapeutic process.
- Honest communication about challenges and progress is encouraged for effective therapy.
Grievance Policy
- If a client has concerns about their therapy experience, they are encouraged to discuss these with the therapist, Katie Lourenco, LCSW
- Refunds for services will be provided after grievances have been reviewed by Client and the therapist, Katie Lourenco, LCSW
Termination of Services
- Therapy may be terminated by either party with appropriate notice.
- Upon termination, clients will be provided with resources for continued support if needed.
Amendments
- This fulfillment policy is subject to change. Clients will be notified of any significant changes in a timely manner.
